Services

• ALink to Resources will provide a list of assisted living facilities, retirement communities, continuing care, and long-term care providers.
• We will provide data on facilities selected (data collected during previous tours.) We will arrange a scheduled tour of the facilities of your choice.
• ALink to Resources will tour the facilities with you and provide you with feedback after the tour.
• We will assist you with the application process for admission and red tape of Medicare/Medicaid questions.
• We will monitor the providers care of your loved one 14 days after placement ensure a smooth transition. On-going monitoring is available for and additional fee.
• Upon request we will provide transportation and aid with moving into the facility of choice. Additional charges will apply
